CCC PATIENT GUIDE
PA Medical Marijuana Card Renewal
Renewing your medical marijuana card involves two separate steps: updating your registration with the PA Department of Health (DOH) and completing your recertification appointment with CCC. Please review this guide carefully before your visit.
PART 1
BEFORE YOUR APPOINTMENT — UPDATE YOUR DOH REGISTRATION
Log in to the DOH patient portal and complete the following steps:
Log In to the DOH Patient Portal
Visit: padohmmp.custhelp.com
Review and Update Your Personal Information
- Your mailing address if it has changed
- Use CAPITAL LETTERS and double-check all info before submitting
Pay Your State Renewal Fee
- Renew up to 60 days before your card expires
- Payment opens within 30 days of expiration — look for the “Make a Payment” button
- If the button isn’t available yet, you’re not in the payment window
Know Your Key Dates
- Renewing early ensures your new card arrives before the old one expires
- Unpaid fees result in your card being disabled approximately one week before expiration
Check if You Qualify for a Fee Reduction (MMAP)
- If you receive Medicaid, PACE, CHIP, SNAP, or WIC, you may qualify for a reduced state fee during DOH registration
PART 2
YOUR CCC RECERTIFICATION APPOINTMENT
Follow the steps below:
Submit Any Updated Documents Beforehand
- Email: intake@cccenters.org
- Or upload securely at: compassionatecertificationcenters.com/intake
- Examples: updated ID, medical records
Pay the $125 Recertification Fee
- Online: cccregister.com/paynow
- Phone: 412-730-3983
- Credit cards accepted online/by phone (small surcharge applied)
- Charges will appear as “Syndikos Investments LLC”
Cancellation & No-Show Policy
- To reschedule, call 412-730-3983 (option 1)
- A $45 fee applies for no-shows without at least 24 hours’ notice
Privacy Acknowledgment
- By proceeding with care at our practice, you acknowledge our HIPAA Data Use Agreement and, when applicable, our Telemedicine Consent Notice. (both available at compassionatecertificationcenters.com)
PART 3
AFTER YOUR APPOINTMENT
Follow the steps below:
Submit Any Updated Documents Beforehand
- The DOH will process your renewal and mail your new card
- For issues with card delivery or DOH registration, contact the DOH directly, CCC and the DOH are separate entities.
